Athlete Registration Form

This registration form is for UConn athletes interested in working with Championship Labs to explore entrepreneurial applications and opportunities surrounding Name, Image and Likeness.

Athletes will work with the lab to create an individualized action plan to monetize their Name, Image, & Likeness.
